Build an intake form and request queue in 20 minutes

Build a Requests table, share a Form view to collect requests, and triage them in a Kanban view.

Written by Peter Novosel

In 20 minutes you build a request queue: a Requests table, a Form view that anyone can submit without a SmartSuite account, and a Kanban view where the team triages what comes in. You need the Solution Creator or Administrator role; forms are managed by Solution Managers.

What will I have at the end?

A form link you can send or embed on a website, a table where each submission becomes a record, and a board that shows every request by status with an owner.

Step 1: Create the Requests table (5 minutes)

  1. On the Home Page, click + New Solution and choose Build from Scratch.

  2. Click Add Table and name it Requests. The Title field is the request summary.

  3. Add these fields with Add Field: Requester (Text), Requester Email (Email), Request Type (Single Select, for example Access, Equipment, Question), Details (SmartDoc), Status (Status), Assigned To (Assigned To) and Due Date (Due Date).

Step 2: Build the form (6 minutes)

  1. In the View Panel, click Create New View and choose Form.

  2. Give the form a title and a short description.

  3. Select the fields the requester fills in: Title, Requester, Requester Email, Request Type and Details. Leave Status, Assigned To and Due Date off the form; the team sets those.

  4. Mark Title, Requester Email and Request Type as required.

  5. Share the form: copy the link to send it, or embed the form in a website page.

Note: Form submitters do not need a SmartSuite account and are not billed.

Step 3: Build the triage board (4 minutes)

  1. Click Create New View and choose Kanban; group the columns by Status.

  2. Submit two test requests through the form link. Each appears as a card in the first column.

  3. Open a card, set Assigned To and Due Date, and move it to the next Status.

Step 4: Share the queue with the team (3 minutes)

  1. Open the solution's dropdown and choose Permissions.

  2. Pick Only These Teams Have Full Access and choose the team that handles requests.

What can I do now?

  • Send the form link to the people who raise requests; every submission lands in the queue.

  • Add an automation later so the assignee is notified when a request is assigned: see the Automations Introduction.

  • Add a Grid View grouped by Request Type for a weekly count.
