A SmartSuite form creates one record per submission; to create a related record at the same time, use a Fillout form or an automation platform. If a submission sent a confirmation email but the record is missing or opens as read-only, check the Recycle Bin first: the record was probably deleted.

Can a form create a primary record and a related record in one submission?
SmartSuite's native Form View creates one record in one table per submission. If you need a primary record and its related records created together, use a Fillout form connected to SmartSuite, or add an automation layer with Make or Zapier that splits the submitted data into related records (for example one line-item record per date, amount and description).
A form submission sent a confirmation email, but the record is missing or locked. What happened?
The record was most likely deleted after it was created. Deleted records are not shown in the table and open as read-only from a link. To find it:
Click your profile icon in the top-right corner and open Recycle Bin.
Search for the record by a known value, such as a customer name or order number.
If it is there, restore it.
If it is not there, take a screenshot of the automation's Create Record action and contact SmartSuite Support so we can investigate.
Which tools extend record creation from forms?
Fillout: forms that create primary and related records in one submission.
Make or Zapier: automate the creation of additional related records when a form is submitted, including parsing one submission into several records.
Frequently asked questions
How long do deleted records stay in the Recycle Bin?
It depends on your plan. See the Recycle Bin article for the retention period of each plan.
Can I link a new record to an existing one from a native form?
Yes, a form can include a Linked Record field so the submitter picks an existing record; what it cannot do is create the related record itself in the same submission.
Who can restore a deleted record?
Anyone who can open the Recycle Bin from their profile menu can restore items listed there.
